The service is delivered under general supervision, meaning that it does not need to be the physician who does the monitoring. It can be an organization, such as US Telcare, who does the monitoring.
CPT 99458 - $47
Each additional 20 min of monitoring time - Each additional 20 minutes of monitoring time within a calendar month. There is no limit to the number of times this can be billed in a calendar month. The service is delivered under general supervision, meaning that it does not need to be the physician who does the monitoring. It can be an organization, such as US Telcare, who does the monitoring.

The physician or provider has initial visit with the patient describes the telemonitoring service. The Provider refers the patient to US Telcare via their EHR.
3. We Interact
US Telcare on-boards your patient and provides required education on the use of the device. We contact your patient throughout the month to provide the interactive time required and will escalate any issues to the clinic based on your defined protocol. Our platform integrates with your EHR and automatically places vitals and reports in patients' medical records.
2. We Send
US Telcare sends the required device (i.e., Glucometer, Blood Pressure, Weight, SP02, and more) at zero cost to your clinic or your patient. The patient will receive a welcome SMS, which includes the hardware tracking number. US Telcare contacts your patient when the device is delivered and walks them through their initial reading.
4. You Enjoy
